Information Security Incident Report

Overview

The Information Security Incident Report service is used to request immediate assistance when an information security incident is suspected or confirmed.

The Technology Resource Center (TRC) investigates and responds to security incidents that may impact institutional systems, data, or users.

When to Use This Service

Use this service if you encounter or suspect any of the following:

  • A data breach or possible exposure of sensitive information
  • Phishing attempts, scam messages, or suspicious emails
  • Malware, ransomware, or virus infections
  • Unauthorized access to accounts, systems, or data
  • Unusual or suspicious system behavior
  • Any other potential information security threat

Reporting incidents as soon as possible helps reduce risk and limit potential impact.

What This Service Includes

When you submit an Information Security Incident Report, the TRC will assist with:

  • Identifying and assessing the scope of the incident
  • Investigating affected systems, devices, or accounts
  • Containing and mitigating the impact of the incident
  • Providing guidance on next steps and recovery
  • Implementing measures to help prevent future incidents

What to Include in Your Report

To support a swift and effective response, please provide as much detail as possible, including:

  • A description of what happened or what you observed
  • The systems, devices, or accounts involved
  • Any error messages, alerts, or suspicious behavior
  • The date and time the incident occurred or was discovered
  • Any actions you have already taken

Providing clear and detailed information helps the response team act quickly and accurately.
